NEW PATIENT MEDICAL HISTORY FORMS:

All new patients are encouraged to complete our easy new patient forms in order for our team to best serve your needs. You can save time and complete this application here before your first visit.

DAY 1:

• Complete Health History & Consultation A one-on-one consultation will take place where we will sit down with you to discuss what health challenges you’re currently experiencing, review your history so we may better understand what has led you to your current health status, and determine your goals for the future.C
• Detailed Spinal Exam: A full spinal examination along with orthopedic testing will help us see where structural shifts and subluxations may have occurred.
• Digital X-Rays/Labs: If necessary, using our state-of-the-art, digital x-ray system, we are able to find any curves, degeneration or loss of disc space in your spine, which cannot be detected during the other evaluations.
• 1st Chiropractic Treatment: At the end of the evaluation, Dr. Christina Wellner will perform your first chiropractic adjustment as well as start your physiotherapy treatments, if needed (i.e. electric stimulation, heat/cold, soft tissue mobilization, Graston, Rocktape)

DAY 2: REPORT OF FINDINGS

• Doctor's Report Of Findings: Dr. Christina Wellner will thoroughly assess your findings, go over your digital X-rays, and present a personalized written-out treatment plan to correct your health concerns leading to your specific health goals being reached.
• Your Road To Optimal Health: Moving forward with your personalized recommendations will allow you to start on the road to optimal health! From here you can enjoy all of what the clinic has to offer including chiropractic treatments, acupuncture, cupping, and medical massage.
